Anthrax nearly kills healthy 18-year-old welder amid puzzling pattern

A new report has emerged detailing a startling case of a rare and potentially fatal infection known as 'welder's anthrax,' affecting a healthy 18-year-old in Louisiana. Released on January 1 by health officials from the Centers for Disease Control and Prevention (CDC), the case marks the ninth instance of this perplexing illness, first identified in 2022. The incident occurred in September 2024, when the teenager, who had no known pre-existing health conditions or risk factors—such as smoking or heavy drinking—began experiencing a cough. Within a week, his condition deteriorated, leading to his admission to an intensive care unit due to severe pneumonia and respiratory failure. He required intubation and mechanical ventilation to survive. Blood tests revealed the presence of a bacterium from the Bacillus cereus group, which includes the anthrax-causing species. Connecting the dots, his medical team noted that he had been working as a welder's apprentice for six months prior, engaging in shielded metal arc welding within the shipbuilding and repair sector. Suspecting an infection related to his occupation, doctors promptly secured an anthrax antitoxin from the US Strategic National Stockpile, a reserve maintained for potential bioterrorism incidents. Alongside a customized antibiotic treatment, the teen showed remarkable improvement, being removed from ventilation just 72 hours later. The swift actions of his healthcare team likely played a crucial role in saving his life; out of the eight previous cases of welder's anthrax, six resulted in fatalities. This report, while concluding with a positive outcome, highlights a troubling and unexplained trend of metalworkers contracting this exceedingly rare and dangerous infection. Traditional anthrax is caused by Bacillus anthracis, which can be transmitted through skin contact, ingestion, or inhalation, and is known for its ability to evade the immune system and produce lethal toxins that lead to cell death and inflammation. In the last two decades, only nine anthrax cases have been reported in the United States. However, Bacillus anthracis is part of the broader Bacillus cereus group, which includes other species, such as B. cereus and the newly identified B. tropicus, that can also harbor and produce anthrax toxins. These bacteria are widely present in the environment, particularly in soils.
