
The National Highway Traffic Safety Administration (NHTSA) announced on Monday that it has initiated a special investigation into a tragic incident involving a Tesla Model 3 that crashed into a residence in Katy, Texas, resulting in the death of 76-year-old Martha Avila. According to Harris County authorities, the driver, identified as Michael Butler, is fully cooperating with the ongoing investigation. Reports indicate that Butler was utilizing Tesla's partially automated driving features at the time of the accident when the vehicle veered out of its lane and collided with the home in this Houston suburb. The NHTSA has been proactive in addressing safety concerns, having opened over three dozen special crash investigations related to Tesla's advanced driver assistance systems since 2016. These systems, previously marketed as Autopilot, have come under scrutiny following legal challenges that led to a rebranding in February of this year. A California court found Tesla guilty of misleading advertising regarding the capabilities of its Autopilot system, which could mislead consumers regarding its limitations. This incident is not an isolated case; a May crash in Clairemont, California, involved a Tesla crashing into a house after striking another vehicle, injuring six people. Witnesses reported that the driver was using Autopilot at that time as well, prompting further investigations. An independent website, TeslaDeaths.com, tracks collisions involving Tesla vehicles and has documented at least 65 fatalities linked to incidents where either Autopilot or Full Self-Driving (FSD) was cited as a contributing factor between 2013 and 2025. The latest NHTSA investigation comes at a critical time for Tesla, as CEO Elon Musk aims to reassure investors about the company's potential to lead in the autonomous vehicle market. Tesla has long promised sophisticated automated driving technology, with aspirations to deploy fleets of robotaxis on public roads in the near future. Despite the unfolding investigations and concerns, Tesla's stock saw a slight increase, closing at $405.05 on Monday.
